Question about the "Sir/Dame" Title

Has anyone pinned this down yet? From what I can gather it's from the quest "Past is Prologue" which spawns from the sanctum messenger after the Kain quest line in stillmoor.

Been sitting in stillmoor for about an hour and yet to see the quest giver show up, (comes from the east and stops infront of Thalin Tor.)

I've done all of the marshal kain quests in the game and still no luck -.-

Any help would be great!

Not entirely sure, myself. Last quest I had involving Kain was the one with Carwin's ghost popping up at the body pile. I'm also like 69/72 quests in Stilmoor, but apparently some are drops from rare spawns that I never find up.
