i would guess that a third of those are handed to you just for completing the campaign, and another third by playing even a moderate amount of multiplayer.

You don't have to guess if you can read. The bulk of them are not Campaign freebies or multiplayer though, and one of the multiplayer ones is winning 500 matches.
