Only thing I recall about SE's OTP was that they were useless for protecting FFXI characters, assuming they ever patched that one rather sad exploit that let you bypass the OTP requirement completely for an account that needed it.

I don't think OTP itself has ever been compromised. Any story out there that claims OTP or 2FA having been compromised usually ends up being a tale of social engineering via some weak link that let them bypass OTP, or the database itself containing the keys being compromised.
