Actually that's not quite true. There was a case in the states where a guy importing manga/doujins was convicted of child pornography.
I cant really think of any cases where anyone in example B) got convicted though.
In the case of manga the characters don't even have to technically "be" underage so long as the court thinks they look like it.

I think the case you are referring to the guy simply took a plea bargain and didn't attempt to make a constitutional challenge. The Supreme Court has struck down bans of visual depictions of minors engaged in sexual activity, including computer-generated imagery and other fakes.

In the case you're speaking of, the manga and doujinshi were actually yaoi and didn't contain children at all.

He was eventually charged with obscenity, I believe.
