I went ahead and filled it out because MI is requesting going by alphabet on certain days and I am up today. For last employer I just put independent contractor and used the occupation code for real estate agent. I'll keep an eye on it and probably call in on Tuesday, which is my alphabet day for call ins.

This morning I logged into my MI unemployment account and an approved determination .pdf showed the "normal" unemployment amount I'll receive. It had listed my "Independent Contractor" job listing. Who knows if a person actually looked at it. As of 12:48 PM the site is dead and I can't review it again.
Wisconsin has a little window that says when they get more information about receiving their funding they'll update. Still no way to file independent.

Other Disney executives will take pay cuts as well, Chapek said in the email. Employees at the vice president level will have their salaries reduced by 20%, senior vice presidents by 25% and executive vice presidents and above by 30%.
Chapek’s base salary is $2.5 million, according to his employment agreement for his new role as Iger’s successor as CEO. He is also eligible for incentive-based compensation including a $7.5 million annual target-based bonus and an annual equity-based long-term incentive grant of $15 million. It’s not immediately clear which part of his compensation will take the 50% reduction.
