I'm starting to lean towards that a certain number of cells need to be used in order for the madame to pop. For future runs, would anyone be able to have their group keep track of how many cells exactly were used?
That being said, would anyone be willing to try and bring a couple mules in their runs, and have them use any extra cells that would normally go to waste ?

OP updated with all current info (and is now freaking enormous).
After examining the op, if the # of pathos counts, then it's somewhere between 20-30. 1 person unlocked = 5 pathos 1 person not unlocked = prolly at least 2-3 pathos from 1F (magic, ability or SJ). Most of the failures seem to have few unlock but it's only eye balling since ppl didnt keep track.
Another possibility is that only equippement(2F) and stats(3F) count, since it's somewhat natural and not time consuming to give SJ+ability+magic (=3 pathos) to everyone on 1F in 6-8 ppl groups.
It would be interesting that ppl keep track of how many pathos and what pathos you unlock.

Things to note: Ares's and Morrigan's hold a very "keep away from each other while being similar" pattern.
Now, with Zhayolm, the relationship between Skadi and Marduk is obvious (though I am quite surprised it was the body piece and not the head that dropped from it). The 4FN Madame drops Skadi, Marduk, and Usukane, which might lead one to believe that's what this Madame drops also. But Usukane doesn't show a close relationship to anything except two instances: 5F Arrapago chariot and 4FN Zhayolm Madame.
I'm gonna go with Arthar's answer for now, Usukane hands. But I wouldn't be surprised if it's one of the feet pieces that drops instead. The former would keep with the pattern of Arrapago/Zhayolm and form a nice grouping of things being beside each other slot-wise (Body-Hands-Legs, Feet-Head assuming an infinite loop).
