They said his "rocket suit" flung around and broke a bunch of shit before eventually making it onto Stark. That hole looks bout the right size for one of the hands or something to punch through. Just a guess, since he looks like hes striking a Ginyu pose after finally getting it all put on, and Jarvis is playing the comic relief.

Kevin Feige actually said something to the effect of "we wanted to break away from the typical Asian Mandarin ..." and something about wanting to have him represent a larger symbol instead of being "the evil Asian guy" which is who the Mandarin was originally supposed to represent.

Remember, most of the early Marvel villains were just caricatures of America's enemies at the time.
