Very old account hacked - form to retrieve?

So as my sig says, I'm an old-school retired player from 2008. I was looking to get back into it and checked my account on ffxiah. It's pretty clear the account has been compromised as its fish botting and all my passwords don't work. Clearly, I went to playonline chat support as a first attempt to get it back and they mentioned some form that they would send me to try and retrieve it. Chat support was terrible, robotic, and they never sent me any form - pretty much the worst possible support possible. I know I can continue to try different people on chat or call, but this is already getting old.

Anyone have any experience with this? Possibly have a pdf of the form or something? Thanks
