The silver skull with green background = BAM, which = group mob. The gold lion with red background you see there = world boss.

Lancers can solo most BAM, but they do it rather slow. You can increase your kill speed by using <50% HP + Crit multiplier damage crystals. Just need to use heals wisely so you don't go over half HP.

That happens all the time, after you initially download your patch. You're not really downloading at 22mb/s.
