Yes, it does.
Min delay is 20% of 480+weapon delay (where 480 is delay without any MA traits at all). 51 delay Verethragna would have a minimum delay of 20% * (480+51) = 20% * 531 = 106.2. You can then calculate the maximum useful haste based on that compared to your delay after MA traits. With MA7, 280+51 = 331. 106/331 = 0.320, 1 - 0.320 = 0.68. So maximum usable haste is 68%.

Is additive in all cases? (i.e.: 0.1 reduction from LA + 0.15 from /RDM for casting time, total 0.25 reduction)

I'm not a monk and perhaps I have this backwards, but I think Melee Gaiters must remain equipped to receive the extra 10% Counter rate during Counterstance. Equipping them for JA usage and unequipping them has never given the bonus.
I have no proof that it is the same for Melee Gaiters +2, but I'm willing to assume it is. The Berserk and Last Resort enhancements aren't activation-only, so I doubt this would be.
Enhances Warcry on Mask is when you activate the JA, but Enhances Berserk on feet isn't (not sure on body, I really can't be bothered to test but I've heard it works like Mask). SE really just did not care when it came to Relic affecting JAs.

Yes, it only needs to be on when you activate the JA. Yes, it's been tested (multiple times, I think, because the question keeps coming up), though the posts are several years old on Alla. Maybe there's a link in the sticky... Hmm.. Looks like Alla might have redone their forum link IDs or something, as a bunch of the sticky threads don't seem to link to anything. I'll see if I can find anything with a quick search..
Here's a thread from 2006 with a miniscule sample size (in initial post; didn't finish reading the thread) indicating wearing or not wearing Melee Gaiters while Counterstance is up gave about the same counter rate, which was 10% higher than without using the Gaiters at all: http://ffxi.allakhazam.com/forum.htm...25738&h=50&p=1
Edit: I may back off on the assertion that it's been tested 'many' times. I'm fairly certain that there's been at least one other test aside from the referenced thread, but don't know where to find it right now. And of course plenty of anecdotal evidence where the drop in counter rate if you forget your Gaiters is quite noticeable, even for those who only macro them in for the JA use. May be worth retesting simply for statistical validity.

Exenterator is best unstacked WS outside for thf/dnc from what I've heard, Shoha is pretty much a given for Sam regardless of weapon, last is probably going to vary.

Pretty much, everyone just blindly agrees with that one old test from alla. I personally don't buy it because it's such a small test, but not enough to make me want to test it myself to disprove/prove it, so I've just gone with it, because it seems to hold.
