although I hesitate to say "can't," you really can't get HIV from receiving oral sex unless you have cuts or sores on your penis, and even then it is an infinitesimally small chance. saliva is an extremely poor vector for HIV.
if you are giving oral sex, the chances are still rare no matter what, but they change depending on several factors. each of the following increases the chances. even with all of these factors present it's very unlikely, but they present enough of a risk that it would be inadvisable to continue with oral sex (except maybe with the first one, which is more of a guess on my part)
- cuts or sores on the penis (mostly irrelevant, but if they are bleeding it could be a factor)
- cuts or sores in the mouth
- presence of precum
- whether or not the man ejaculates into your mouth
- presence of other STDs (this is an interesting one... did you know that the presence of other STDs will increase the amount of HIV in a man's ejaculate? yep, it's true)
keep in mind that cuts and sores on the mouth are not always obvious... it is fairly normal for your gums to bleed a small amount after brushing, so small that you wouldn't even notice, but enough to open up a vector for transmission. in fact, giving an HIV+ person a blowjob after brushing your teeth is actually not recommended, go figure.

She can look into a ParaGard. It works fine and hormone free. Can stay in up to 10 years., and after that, you just replace it with another one. I have a Mirena (that's a hormone one though, so lasts 5 years) and it is literally the best thing I have ever bought. Worry free, can't feel it at all, and Ryko has never been able to detect it. So yeah. Awesome.

If you actually trust your wife, then maybe you should try trusting that your relationship will be just as strong as ever for taking the health precaution of getting tested. It's not THAT expensive either, nor does it take much time. Heck, sometimes people are infected by other means than sex, such as by birth or blood to blood transmission, or by an infected needle by some horrible doctor, or the like. And it's not like HIV is the only thing it tests for, if i recall correctly.
It's like anything else in life. Do i trust that I'm not going to get struck by lightning indoors? Yes, but that doesn't mean i go out and check out the lightning by standing right at the fucking glass, just in case one happens to actually strike right at my feet.
Trust does not mean you cannot be smart and cautious as well. If you care about your own health and that of your wife's, you will get tested to MAKE SURE that you're both not infected. Not because you don't trust her, or visa versa, but because you love and trust each other too much to want to see each other possibly hurt.
