I know some drag queens, really just one on a first name basis (Shirley) I've never met her out of drag but I always (after my first mistake) called her she and I never cared if she was straight or gay
transgender I have zero experience with, but by default if they are attempting to look female I would say she out of respect until corrected. If you look like a dude and I had no idea that was supposed to be a female presentation, relax, how could I have known (get a better doctor? I don't know).
