I don't see why they shouldn't be on at all times. The technology is easily there. First complaint would be that they'll run out of battery or it'll be unsafe for the officer to do that.
Charge them while in the patrol car. Safety hazard from plugging and unplugging? Nope, there are breakaway chargers that wouldn't inhibit movement. See MagSafe adapters by Apple.
Problem is storage. Even a 256gb Micro SD card would get filled up before the end of a shift as most if not all body cameras are filming at the very least 720p. Then you have network storage. Our county required any and all video recorded to be kept on the network and accessible at any given time for subpoena for 5 years. You do the math on that storage, that bill is going to be fucking astronomical.
That said, I know the technology is out there to have it where if an officer activates their lights in their car, it turns the camera on automatically on dash cams. I'm not sure if that same tech can apply to body cams. But yes, at the very least it should be mandatory, you get dispatched, a civilian comes up to you, you see something suspicious, that needs to go on immediately.
Now also that said, if you are on foot patrol, and the unlikely event of shots fired happening near you, are you really expecting an officer to fumble with a body cam before drawing to pursue a suspect. An easy way to resolve that if it's possible is if an officer responds on radio to an incident and they are in pursuit, if they cameras can be turned on remotely by dispatch or someone else so the officer can focus on what they need to do.
The tech is already there to turn on cam and mic when their weapon is taken out of the holster. Some departments use it already.

It's not 24/7 first and foremost because cops don't want every single conversation they have to be recorded.
Honestly it would suck ass to not be able to vent about your boss to your partner because you know everything you say is being recorded.
And contact with civilians should obviously be recorded.

I dont know why they dont impose "ignorance is not an excuse". If something happens and they "forgot" to turn their bodycam on, too bad. If i "forgot" to stop at a red light, i dont get forgiveness because i forgot that red means stop. If i "forget" to pay my rent, i dont get forgiveness for it because i forgot to do that.
Its part of their fucking job.
