Mine does that too, even though it came with windows 7 default. Something I installed later triggered it, but I have no idea what.

Reinstall your gpu drivers. If that doesn't fix it, run memtest86+. If that comes up clean, try a different GPU.

Definitely hardware. Not the OS.

Both of those examples appear to be concave. Unless buttons are somehow magically different from lenses, or I'm seeing a weird optical illusion.

i like to hold the door just enough so that they're barely out of reaching distance and then leaving and letting go so the door hits them

I have a tendency to just automatically hold doors, whether there's people around or not. I was super tired and not feeling well one day, stopped at the gas station for something or other, my friend was working (No other customers in the store). We talked for a moment, I went to leave, and stood there for a good 10 seconds before he asked what I was doing. I realized that I'd been standing there holding the door for him. Was hilarious.

I do the reverse, I let them get into the doorway and if they haven't said thank you at that point I let the door go on them.
