The ground floor and first floor bit? It's pretty common.
Project Ogre has been in production for a long F-ing time. Hope that means this sees a 2013 release date or at the very least early 2014.

This would support the belief that the “V” in “Metal Gear Solid V” stands for “Vita,” as the Metal Gear Solid series typically numbers its games with numerals. But that’s just speculation.

I dunno. If it's just some side story about Liquid/Ocelot, it wouldn't be too bad if it was on Vita. Especially if Kojima is behind it. Look at how good Peace Walker is and that's on PSP(well, 360/PS3 too). We're already getting Ground Zeroes for consoles, so it wouldn't bother me.
Edit: Although, if what I said above is Project Ogre, that will be somewhat disappointing.
Unless the entirety of that teaser trailer was bullshot, I don't see this being possible at that graphical level on a Vita. The self-shadows, the lighting, the smoke effects.....it'd be too much for it.

In the UK every building starts from Ground floor then goes up 1, 2, 3... while in America, Japan etc everything starts from the 1st floor.

Japanese gaming magazine Famitsu is teasing “something surprising for Metal Gear Solid fans” in its next issue.
The issue in question is due to hit shelves next week. Could this announcement have something to do with The Phantom Pain which was revealed at the VGAs last Friday? Based on the timing of the announcement and the sheer amount of Metal Gear Solid related hints found in the trailer it stands quite a good chance.
Several fans believe that The Phantom Pain is secretly a Metal Gear title for several reasons which are analyzed here. There’s also the fact that developer Moby Dick Studio’s CEO Joakim Mogren’s first name is actually an anagram of Kojima.

Haha, noooooooo! I really do wonder though. Doesn't anyone else have that fear in the back of their minds? The other games shown in that 'hacked but just kidding it was a mockup' Wii U community (Yoshi's Island, DQX NA release) have been pretty much confirmed haven't they?
