Imagine you're on first base, leading off for the usual tactical advantage. The pitcher starts the same windup he's been doing all game, you plant your feet to take off, and he throws it to first instead. Misdirection isn't really allowed like that in baseball. You hear about the Hidden Ball Trick, where a fielder fakes returning the ball, waits for a runner to lead off, and tags them, but all of that is contingent on the pitcher not returning to the rubber, else a balk will actually be called in this case too.
Every pitch is initiating a play, just like a hike in hand-egg, and the previous play is still ongoing unless someone asks for and/or time-out is called by an umpire. The ump calls a balk when the pitcher is being a dick and fucking with whether a play has ended or begun or not. True story.
I found out that if you stare at his eye, the worms will march in both directions.
