Am I the only one that uses Vats to see what enemies are ahead, or where mines and shit are? Whenever I explore I hit VATS, like, every 100 feet just to know if anything is hidden. I rarely use it cept for when my Crit meter is full and do some nice Damage that way.
I spam the shit out of VATS when I'm in a ghoul area.
Yeah the idea for the difficulties is that you should switch them progressively as you get further in, there are no trophies/achievements related to difficulty. Starting out above normal is retarded since enemies are bullet sponges but ammo and other resources are scarce, this is made even worse by how enemies come in hordes and early on you mostly fight ghouls which are a resource syphon. Early on you also go to that raider hellhole where they throw endless grenades at you, heal themselves with stimpaks and snipe you from the roof with shotguns.
If you enjoy that though then good for you.

got an annoying bug. when I click E on Piper, she talks to me like normal but the companion menu doesn't come up, thus I can't recruit her or access her inventory. guess that leaves the only other female companion in the game.

I fixed the Piper problem. Experimented with some debug commands I used to use when making Skyrim mods. If you ever get a follow stuck or stop talking to you and are on PC, open console, click on the follower, and type this:
removefac 00075d56
This removes the Player faction from the actor (npc) list and lets you talk to them as normal. You can then add them as a follower again.
I have a weird bug with the minuteman guy from the first town. He Follows me But hes like sitting down on the ground the entire time and sliding around shooting like that. Also he wont trade me shit.
Have people been building walls around settlements for safety or just letting them be for the most part? Any settlement I'm currently building up, I make sure to build steel/wood walls around it while only leaving 1-3 doors that are guarded by a shitton of turrets.
So i started a new save because i made some poor perk decisions and accidentally stole something in diamond city. i'm enjoying it more on hard than i did on normal, even though some stupid raider stole my god damn power armor in the assembly plant. haven't noticed many more legendary enemies on this playthrough but again i'm only around level 12.
