Some gear swap gear, maybe a ring or something, otherwise all replaced. They're turning FFXI into a modern MMO with constant gear replacement and ilvl increases.

(As for your question, though, you will not have to completely start over—macro-able items for certain abilities will still be relevant, for example. Fortunately though, base-level gearing is easy now; you can look up events on BG Wiki like Skirmish, and then get into a group and use them to gear yourself fairly quickly. If that seems like too much trouble, read up on the Bayld currency and how to get and use that.)
