There is little to no evidence that back surgery (50% significant reduction in pain at 2 years) is more effective than PT (41% significant reduction in pain at 2 years), and neither is probably much better than literally doing nothing (25% pain free after 1 year). Similarly, there is no evidence that vertebrae fusions improve function or reduce pain, but all these procedures are very common.
Our best treatment for neuropathic pain (Gabapentin) probably works less than 15% of the time, if it works at all.
Opiods are over prescribed to vulnerable populations (like seniors with dementia) because doctors can't be arsed to ask more than "rate your pain on a scale of 1 to 10" and anything over 5 warrants opiods in someone they trust to not abuse them.
I've been taking a class with clinicians for the last few months and it has been eye opening. Doctors choose what they recommend for you based on a huge collection of anecdotes. If you've ever asked a doctor "Why?" and thought their answer was lazy, know that they probably answered it as well as they could and just didn't have a better answer to give. Out of the clinician lecturers we've had this year, I've had only one that I would trust to make decisions for me if I was unconscious and she wouldn't be qualified anyway because she was a gerontologist. It's really fucking bad if you talk to them while knowing anything about the science.
I trust my PCP who was a military combat medic for like 8 years before rejoining the civilian world and completing all of his doctor training. My endocrinologist is a tard though and sees no issue with 'treat type 1 diabetes for life' as opposed to 'cure type 1 diabetes', refuses to entertain any possibility that big pharma has an influence on the drug/procedure approvals/denials at the FDA, and has little knowledge of the stem cell research progress in curing the disease. Unfortunately there's only like 6 in my whole county for a million+ people so I don't get many options.

At the same time, though, I wouldn't recommend anyone ignore the advice of medical professionals and self diagnose. As many stories as there are of retarded shit doctors do, they have hundreds more stories of people they've seen that felt too comfortable taking control of their own treatment. Noncompliance is a huge issue and causes way more problems than unnecessary back surgery.
