The "alt-left" has existed for a loooooooooooooooooooong time - in fact they were most visible during the 2008 campaign. Remember the LaRouche idiots who were the first ones to run with that picture of Obama with a Hitler mustache? Then there's the more militant Marxists, Socialists and Greens that are present at pretty much any protest action I've ever participated in since high school. And the more mainstream leftists have always thought they were just as politically insane as the Republicans.
