It's been a while, but iirc when I looked into the subject, my impression was.

Yes that's literally exactly how it works.

I used to rent kayaks from the East Coast launch detection site in Cape Cod. The US missile defense defense has been a thing since the early Cold War, but for obvious reasons a lot of what goes into it is classified.

To add, detonating a nuclear warhead is not just a matter of having the missile hit the target, there's a lot that has to go into the process of generating a nuclear explosion that has to happen before one gets the massive explosion people associate with them. Intercepting missiles is incredibly complicated, but taking it down before the process has completed turns into more of a local radiation hazard then an extinction level event.

Russia and China could definitely hit mainland US. UK and France could definitely do it too if there was some bizarre reason for them to want to nuke us. North Korea *may* be able to hit the west coast but leaning towards no. It’s really Russia and China having nuclear submarines that gives them their biggest advantage. With enough warning missiles could be intercepted but the ones being fired from subs give much less reaction time. I’d prefer to not have to find out.

I'm pretty sure that the nuclear states in the security council all have guaranteed second strike capabilities for strategic level weapons through their subs.

And most have jets on alert status that can get up into the air with a payload between the window of detection but before the first strike hits.

To harken back to the earlier question about missile defense, our odds of taking out an ICBM are at best 50%. Russia has a lot more than twice the number of ICBMs it would need to glass us.
