Guess I should have - the whole conversation between Charlotte and Theresa where Charlotte said something to the effect of "No, it has to be someone that's a complete surprise." In this interview, the producers explicitly stated how this happened.

Fucking love this show. Just caught up with all of it. It's like part Alice in Wonderland, part Deadman's Wonderland, part Deus ex Machina, part Inception, part a bunch of other shit! Like it took everything best and made it a show. Except for William. That dude is fucking pathetic and needs to go away.
