It doesn't always though. Examples with no documented history of mental illness and being cleared by psychiatric twice had my neurologist threaten to get me committed if I didn't go to some group therapy.
Or how about someone I was in nuke school with. While what happened was a bit more extreme (lost track of about a day and ended up with minor burns in the shape of letters in his arm) it wasn't exactly his fault. They had proscribed him a cocktail of various psychotropics for his insomnia and migraines and threatened to write him up if he take them... and then all of a sudden they realized he can't be on any of that and still be in the navy so they made him go cold turkey. While still kind of out of it he got told volunteer or be volunteered. He volunteered but got entered as involuntary. Whoops. Funny part was I went in to medical for the same stuff but got told to fuck off because I had .1 higher gpa so I was "clearly coping well enough"
Or another guy who had been shit on by the entire chain of command for I shit you not being Canadian. Or more specifically having dual Canadian and US citizenship. All the stress effected performance which made him have to stay late which stressed relationship which meant even less sleep so he was kind of always out of it. My ex actually reported him as being suicidal and next day gone not a word.
