It doesn't really matter. Issuing a fatwa is a global thing. Now, given it's an Iranian cleric, it's going to have less of an impact because Shia clerics are a minority. But in no way is this restricted to Iran.

Fatwas being issued against people have a long history of violence. It tends to follow those people for life. Salman Rushdie is a famous example.
