There's a short book called Down and Out in the Magic Kingdom. Had to read it for a college course in Fantasy, Sci-Fi, and Religion. Was a fun, quick read. It's about consciousness transferring and the main character experiences a glitch of sorts and he's unable to transfer anymore.

Not glitch, but damage to the implant that enables it. And he can transfer, but doing so would mean losing memories of a specific timeframe (last backup to repairing it), which he doesn't want to lose.
