We don't actually know. We know what is said in the movie, but also know that everything the machines allow humans to have or know is explicitly for control. Other versions of the matrix also existed before the version shown in the movie where other unusual programs escaped from. The version in the move has also had multiple resets.
I assume the nightmare matrix was medieval or Victorian.
