No idea what led to it, but there is a video out there showing when it happens. You can't see much, just a bunch of people crowded together and then a gunshot. Next thing you know there is a woman laying on the floor with a bullet in her neck bleeding out.

There is another video where you can see what happened more clearly. She was climbing through the busted window of the door they were trying to breach then you hear a gunshot and see her fall to the ground.
