    The Station Six and Welrod are manual action pistols. When he fires the first shot, you can see a slide go back and a puff of gas from the ejection port, so it's 100% not either of those. Based on the malfunctions and the moving slide, it was almost certainly some sort of Browning action pistol and more than likely a homemade suppressor, but possibly also a heavier rifle style suppressor without a Nielson device.

    Internet has been a wild and amazing place today though, some really killer memes and jokes.

    Anesthesiologists lobby CMS for better reimbursement. We all do. When they threaten to cut back more because a group over bills (and they know) the group can either stop taking the insurance or charge for what is leftover because they need to (such as in my case, I either discharge a patient after 3/5 days of treatment or charge them the extra 2 days because they need it).

    The end result is actually worse care if a practice group is doing it right because they're forced to cut corners in order to get properly reimbursed for their time. So they either charge more (what usually happens) or they make less.

    This crap where insurance companies go LOOK YOUR DOCTOR TRIED TO CHARGE US $900 FOR A SALINE BAG well yes because I'll only get $100 for it anyway but if I charged $100 guarantee insurance only pays me $10 and the patient still pays $90.

    Insurance is a fucking scam and the anesthesiology groups that were committing essential Medicare fraud should be ashamed.

    Forgive my ignorance of American healthcare, but in the example of a $900 saline bag, if you're only getting reimbursed $100, where is the other $800 going and who's paying the $900 total?

    No one pays it. It's a negotiated rate. For example (huge hypothetical amount) I might charge $1200 for a 2 hour intake. Insurance will only pay me $300, leaving me to either bill the patient $900 or take $300. Clearly I'll take the $300 and not bill the patient but maybe $125.

    But if I billed $425, they aren't going to pay me $300. They're going to pay me like $125, which leaves me to either bill the patient $300 or take a hit because less than that won't keep the lights on if I have to keep doing that.

    The explanation of benefits to the patient will say "$1200" no matter what I'm paid (and they will tell you they only paid me $300) but in reality the patient still pays me $125, which is reasonable, while the insurance collects another $50-500 in premiums from the patient and only pays out one $300 payment.

    Insurance is a joke. They literally exist as a means of being a glorified bank to hold on to as much of their customer's money as possible while getting them to blame the services they need it for.

    The other issue is CMS tries to dictate what they think is appropriate in terms of reimbursement for work as "cost control" but it doesn't actually control cost because the real cost of health care to the patient is their medications and any major procedures they get. I make zero money on prescriptions. I only get paid for my time and opinion. Surgeons, anesthesiologists, and specialties that do procedures get mad because their material cost doesn't decrease (it actually increases) but their payment does and the work is the same. The patient loses out and the insurance company STILL pockets the same amount.

    Does that make sense? Your premium cost increases and their payment reimbursement decrease. Where do you think that goes? The doctor?

    One of the first things I thought, is that particular hostel contacted NYPD right away saying "Wow, we had someone here who looked like him". Then the police rush to push out this photo who potentially is not even him.

    Or maybe it's just a different set of similar clothes. Guess we'll find out soon. Or not.

    You can't have this conversation without agreeing that people like the victim are using administrative decisions to effectively kill people. For some reason, people think that's okay.

    If the CEO denied claims by shooting patients in the head, people might understand. But because he did it with administrative paperwork, they think that's a-okay and deserves zero ire or punishments. I disagree, but then again I'm a filthy leftist.

    No, because the system lets them just murder people vicariously through spreadsheets instead of stabbing them in the throat. President is liable for murders carried out by drone strikes they issue, Kings are responsible for the deaths of a hamlet they sent their knights to, blah blah blah.

    If this guy gets caught, I'm gonna guess it'll be because someone rolled on him. He left town for 10 days, likely has a tragic backstory filled with friends and family knowing his situation, and his face is all over the internet now.

    "Oh man, that dude looks like cousin Dave, who lost his wife last year....oh hey, didn't he say he was taking a business trip out of town for a couple of weeks?"

    He's reached hero status. Now does he rise to martyrdom?
