schizophrenia onset is clustered post-pubescence to early 30's, with men trending younger and women older (and as always individual outliers, one of the sadder things i ever saw was a 9 year old girl with schizophrenia). if you're saying this kid couldn't possibly have schizophrenia feel that's not a diagnosis anyone should be attempting to make right now with the limited information available.

Because he was a grunt the VA would only allow him to do "bolt metal plate to spine and gg" procedure where as officer ranks got the actual surgery to try to fix it. Obviously different in some way but I would imagine UHC lowballed him or something, or maybe he just went rando on the CEO and no real thing there to look back at, either way don't care. Fuck the CEO

Back surgery outcomes are bad... a lot. I have never bothered confirming this, but I was told in grad school that after your first back surgery, the average time to your next back surgery is two years for the rest of your life.
Try everything else before any of you start down that path.
From what I've read from other other people, and from what orthopedic surgeons explained to me, only really consider surgery if your back is giving you significant quality of life problems because that first surgery likely won't be the end of your problems.
I've chosen to just self manage my particular scoliosis because of being warned of high probability back pain after surgery for the rest of my life. More than any mild pain that occasionally pops up now if I don't at least occasionally exercise and stretch.
Adjacent level disease, the range can be 5-20 years before they have to extend a level above and/or below. Also don't go to an orthopedic surgeon for a fusion, that's just asking for trouble. Go see a neurosurgeon. Epidural steroid injections only work for so long plus they have their own set of issues with chronic use.
In short: the human body is a mechanical nightmare
This trial has potential to be incredible. Dude is already yelling at media that all of this is an insult to the intelligence of the American people.
