Heres an idea.
Gilgamesh instead of Culex as a cameo fight. Came to the Mushroom Kingdom looking for the fabled sword Exor.
When SMRPG was released, Gilgamesh's only appearance was FF5 which wasn't released stateside until the PSX.
Keep the Culex fight, sure. But this would be peak to have Greg appear on the bridge to Bowser's Castle and challenge for the right to battle Exor. Tie it to a sidequest involving the Axem Rangers and how someone keeps pestering them about swords when all they use are axes.

The worry over physical came from the announcement and initial listing on Nintendo's e-shop only had digital for pre-order. Most retailers now have physical available for pre-order.
The Mario wiki, which pulled details from the Nintendo press site, says that Nintendo has yet to reveal who is the developer. SE could simply be just the copyright.

Yeah honestly I see secret fights being the most likely option for new content. The main story is already quite tight and honestly doesn't need padding.
Honestly my only ask is unlimited inventory. That was straight up SNES memory limitations. If they otherwise do a 1:1 faithful reproduction of the original in Switch 1080p they'll make a fortune.

Something with how the SNES read frames I think meant that there was no consistency as to when to hit the button. It could be 2-3 frames in either direction for Super Jump to get the continuation on one press, but different for the next one. Made it very difficult.
As for the developer, best deduction so far is who it isn't. Grezzo seems off the table apparently working on something Zelda related thats a while off (best guess is Oracle remakes). Next Level would probably be tagged with Luigi's Mansion Dark Moon. Intelligent Systems seems probable for the Peach game that was revealed.
ND Cube (Mario Party) seems like the best guess right now, potentially with some influence with former Alpha Dream staff (the style looks similar to Superstar Saga) and potentially even Monolith's support team in there now that Zelda is done. SE's involvement is unknown, and could honestly be on either end of the involvement spectrum with "here your characters to use, les us know if you need anything else" to "we want to be active partners on this".
This game has leaked. It's out in a week, so be wary of spoilers til then (apparently there actually are some).
