When KS99s first came out, they were really rough for a lot of people. When Kirin first came out, he was a big deal for most LSes. When the wyrms first showed up, they were impossible dreams for HNM teams.
When the stuff first comes out it is as hard as you say, but people learn strategies and adapt and eventually they become really easy. So even if you do add something really hard like you want, it is only a matter of time before you're posting this same exact thread over again.

More stuff that allows smaller groups (2-6 players)... Fun stuff like the new "Apocalypse Nigh" quest.
Quests with rewards besides gil and XP scrolls. Maybe Questable Nomad Moogles in Selbina and Mhaura. Questable mog house expansion (multiple safes, multiple rooms). More control over Moghancements. Can't stress that last one enough. I like to keep actual "furniture" in my mog house, and display the fun stuff like nation flags and mannequins. But I'm stuck with a crapload of tarutaru stools all over the floor so I can level clothcraft 88-98 (lol 100) without blowing my gil wad.
MNK JSE? I would KILL for a piece that had "increases 'Guard' activation rate" on it... Skilling up Guard is damn torture. Maybe a new job ability "Guardingstance" where you lose all ability to kick, counter, and you only swing with one fist for half damage... but your "Guarding" rate goes up to a similar rate as Counterstance. My guard skill goes up 0.2 points and I almost feel it's worthy of starting five new threads on Killing Ifrit to celebrate. It's that bad.
Three zones similar to Limbus; basically taking place in an alternate reality version of Oztroja, Davoi/Ghelsba and Beadeaux/Palborough. It'd be possible to obtain rare/ex versions (maybe even HQ type versions) of CGF monopolized NMs like Mee Deggi. Other drops also, obviously. Completing the zones would grant a rare/ex item to an instanced fight against Fafnir (Yagudos), Behemoth (Orcs) or Adamantoise (Quadavs)... each instance would have a 10-20% chance of dropping King instance items.
Toss in two new Limbus zones... One Temenos (only sorta outside) and one Apollyon. Hard as hell, but chances to pop stuff like Serket, Cassie, Simurgh and Arthro.
But that's mostly the ramblings of a bitter old player tired of camping against the damn Chinese gilsellers and dumb-as-fuck NA Vokebots.
You know what I meant in my post, I don't know of anyone that doesn't want to fight something challenging here and there but to have something come out of nowhere in zones that can already be dangerous would piss more people off. If you think I don't have any experience with some of these mobs then you're mistaken.
Just imagine for a moment Ulegurand (sp?heh) Range where Jorm could come flying over you in the sky and aggro you while you're trying to get past some of the IT demons and Tauruses on the way to exp. Is your idea of a challenge the chance to be aggroed by a giant dragon in the middle of mobs that can quickly tear you apart? I'm saying this although Jorm already sits in a large clearing with some bombs and the common ice ele pops around him but most groups are prepared to deal with that. I could understand if you were suggesting Jorm and Tiamat be flying around in the areas they already stay in and coming down when they aggro like the yovras in al'taieu but there is no "challenge" if a mob that's meant for at least a full alliance can aggro anyone, anywhere in the zone.
Oh and about my last comment, I dunno about you but camping 3 different monsters against about 600+ people with a 75 job isn't what I'd call fun. King camping would probably still happen even if abjurations ever got added to KS99s, I'd just like to see the possibility of them dropping off their respective counterparts to ease up the sheer amount of people that can be at any given king. I'm not going to lose any sleep either way, I'd much rather get a Valor surcoat than a Koenig body. XD

Cross server ballista
Have Devs and Test players play this game for three months as four - five HNM LSes. Farming Sky and Camping HNMs. Doing Dynamis, mainly CoP ones, as well. Maybe make two of the teams play in USA to add realism. The team with the second lowest amounts of drops, watch their family die and are murdered a few moments later. The team with the lowest amount of drops have to continue playing the game. That'll change a few things.
SE needs to create more situations that cater to the story lovers(NPC, CoP) and the drop lovers(Sky, Limbus) at the same time, but stop releasing stuff that neither like(Sea).
I'd like to see some new Fellowship quests, and the possibility to expand their armory a little more. It'd also be nice if we could have more conversation options when talking to them. Perhaps a new fishing ability for your NPC so they can fish alongside you, I think that'd be pretty darn nice. They could also take part in the fishing contests against other NPCs and we can see who has the best NPC. That'd distract us from all this HNM nonsense.
I'd really like to be able to use them in sky/sea, though they have made it impossible atm I guess because of the Gods and whatnot. It would be soooooo nice to be able to exp them on Flamingoes or Hpemde though.
I can see SE doing that. Instead of addressing real problems in the game, they'll make more adjustments to the fishing system. Mage weaponskills that actually have a use in battle. More things like starlight/moonlight. And let mages get tp by casting spells instead of attacking. WS like +mp healing, INT or MND boosts, weak refresh, etc....
