Hollow as "strong" as the grand fisher dont exactly pop around often. Then theres the fact that his wife probably knew that running the fuck away from giant monsters was a good idea.
Basically Isshin hadnt counted on:
A) the grand fisher's decoy ability
B) Ichigo falling into said trap since the mother would have known the "ghost kid" in danger was already dead and as such couldnt get hurt.
There's also the fact that he had a RL job and eventually he was bound to stop being paranoid and trying to sense hollows everywhere. There's even a Shinigami watching over the town so even more reason to be relaxed. It's also possible to sabotage shinigami intelligence etc as shown by mayuri capturing the quincy. Aizen's shikai alone can ensure that any hollow specifically sent by Aizen will be unnoticed if you thikn the "just as planned" line was true.
Also consider how bleach started out. Isshin probably had something limiting him all this time such as a gigai that you cant take off for x amount of time if the damage taken wasn't just an act. Keep in mind Ichigo had never actually seen an actual hollow before chapter one which indicates that Isshin was probably killing most of them before they got anywhere close to prevent anything happening a second time.

He didn't flip Aizen off, as nice as that is, he was actually releasing the string of his bow, that also serves as his sword
Aizen was blown away by whatever attack he used with his bowswordthing

edit: also it's pretty obvious the big revelation about Ichigo is that he's a human/shinigami hybrid, since he says "the answer is right in front of me"

For right now at this moment until Kubo screws something up he has beaten Kenpachi too.
The way Aizen says "Because you are a Shinigami and a-" in chapter 397 page 18 sounds like the 2nd part is the 'special' part. 'Human' just doesn't sound that special. We all could've guessed he's human. The way he words it sounds like he was gonna say something else like Hollow or Quincy even like someone mentioned.
...but then this chapter happened lol. Now it's human and something else, so bleh.
Yea, gotta say that I've felt really refreshed about this week's chapter and future chapters.
Just as long as Isshin doesn't die, get his ass whooped, or Aizen has another Just-As-Planned moment.
I gotta say, one of the few things I really missed from the early early episodes of Bleach, was the comedic relief between Ichigo and Isshin. This was really pleasing to read.
Here's the MS translation clarification page mentioned above earlier. The way I see it, Kubo gave us an epic chapter, but just couldn't release it unless it had some form of trolling, so he swapped Shinigami with Human.

I guess we'll have to wait for Kubo to confirm what he meant since it's impossible at the moment to tell which is the proper translation since the mistranslation came from Jump itself. Going with he meant Human/Shinigami myself since Ichigo stated the answer was right in front of him, Isshin. Loved this chapter, hasn't been said enough but Isshin is a fucking boss. The headbutt line is one of the single best comments of the entire series.
Re-reading through the first couple of chapters of the manga I noticed Isshin got his ass-whooped by the very first hollow to attack in the series. Then Rukia supposedly erases his memory of the event. Which leads me to believe, Isshin's power must have been completely restored at one point after the SS arc instead of it gradually returning after Masaki's death. A weakened Isshin shouldn't have had an issue with a low-class Hollow, Chad was able to fight one off when he couldn't even see it in the beginning ffs. How or what caused Isshin to regain his powers back so suddenly will be interesting to find out if Kubo ever goes through his back-story.
