Did they ever say they've been in Karakara town the entire time? They could have been living somewhere else and recently moved here when they found out about Ichigo. I'm sure it's something Kubo never thought through, but it could be a possible explanation.

While I joked about my European medieval soul reaper idea, the premise that there are other places in the world where you'd probably see Hollow and other such activities would be the most sensible reason for people to be elsewhere and at least converge upon Karakura eventually due to it being more of a spirit hot spot. I mean, we know Don Kanonji is pretty much a hack when it comes to actual Hollows, but he is at least spiritually aware and has basically done his thing traveling around for his show. I think he's supposed to be American, too.

I'd think the people of Karakura basically losing a day to the sleep/transport would be enough to get other people interested in the city, and they had 18 months to mosey in, set up shop, and scout Ichigo a bit. Plus we have no clue what all Chad told them once they arrived.

Bleach is one of those manga where my eyes move across the page and, finding nothing interesting, fail to record any information to my brain. After reading this week's chapter in this state, I said 'wait, what, did something happen? I can't remember' and I read through it again to confirm that indeed, nothing happened.
