Personally I have mixed feelings about this outcome, Byakuya has grown in character since the start of Bleach, and his entrance to the fight was pretty badass, I've come to like him more and more with each arc (his speech as he puréed Zommari was great imo). However Byakuya being dead has many interesting potential developments. Rukia would possibly become head of the Kuchiki house, Renji would also be the most likely person to avenge his death, given that he's already familiar with how Senbonzakura works (I refer to the early fight against Byakuya, where although he was beaten, he was commended on being able to scratch Byakuya's face).

Alternatively we could have seen Kenpachi avenging him, since they've had an ongoing rivalry since the SS arc, and with Byakuya dead that'd never have been able to be resolved, so Kenny would have been able to partially resolve the rivalry by fighting a Pseudo Byakuya (As Nodt with Senbonzakura). Also take into account Kenny's natural advantage against As Nodt's fear theme.

tldr; Like it or not, Byakuya being dead would have been far more interesting from many standpoints.

Yama took out Nodt, Bananananana and the dude with the mohawk with his Shikai, however we later saw mohawk dude get back up with minor burns. I'm baselessly assuming that the other two may have also survived.

Was this panel ever explained on who it was? The page before Hiyori states she "smells shinigami" and it's not the two new shinigami's Ichigo saves from the hollows early on in the arc. Wondering if it could possibly be the same coalition of people who saved Urahara, Hikifune appearing before Hiyori maybe?

I don't think it was ever directly stated. If you wanted to go in a roundabout way you could say that because he achieved bankai and every captain with the exception of Kenpachi has achieved bankai his spirit energy is of captain class

Byakuya said when Renji first tried to fight him with Bankai that it usually takes another century after achieving Bankai to attain proficiency with it. Renji is obviously stronger than most other Lts, but he'd easily be the weakest Captain if promoted.
