He's covered in blood a la traditional bleach style. It looks more like the sword went to the left side of his neck and possibly grazed him as there's no blood spewing out from the front or back of his neck. I think the blood on her cheeks is either from that or from him coughing up blood.

yea was a bit rushed small shit here and there "I praise the will, fact that you removed your eye patch" pretty sure they meant to put "I will praise the fact that you removed your eye patch"
and it threw me off when she said "your at your bottom" i was like "wait isnt he stronger when the eye patch is off.. Wouldn't it have made more sense to say "your at your limit".

I'm pretty sure that this is an homage to back when Ichigo first fought Ken. Unohana is basically saying that Ken can't die... every time he comes close his will to fight "brings him back" and he cheats death.
What would make sense is if his sword's ability is an ironic ability to heal himself in order to continue fighting and living another day. Perhaps it's the same ability as Unohana, and she has just adapted hers to heal other people (which is why she never has used Bankai).
Either way I like the direction this is going now.
Could be Kenpachi's sword ability, but that would make this quite lame considering his advantage is supposed to be innate strength. They could rig this by saying "oh, but he's using Shikai!", but that would still be lame lol.
He coughs up blood as a result of being stabbed in the neck (and the picture is pretty direct in showing it go straight through the centre), and a drop hits her face. It's still there after the change up, intentionally to show that it was a real deal I guess.
I imagine he died and she healed him, and then set the situation up to go "oh, you nodded off there for a second, what you doing lol?" when actually it was probably a few minutes of blackout.
Perhaps it's Unohana's bankai that allows her to resurrect him, who knows

Looking for a third time, the picture still doesn't show that. However I forgot to look back at the previous pages when I went to look a second time and using those as context you're probably right. I guess kubo just got lazy and didn't want to draw the blood that would occur from a stab through the neck.
It was pretty blatant lol. Kubo makes it a point to show a close up of her face with the blood still there.
I was originally just talking about the single page in which the stab occurs, in which it is most definitely not clear. It's not like that matters at this point anyway, once you include the previous pages (which I said I forgot to do during the double take) it becomes more obvious.

Her sin is Zaraki. She killed him in the past but brought him back with her Bankai. Something went wrong and now he literally cannot die, he just returns stronger each time.
That would be kinda lame. People don't die enough as is in Bleach.
I think it was pretty clear that what happened with the sword through the neck was the same thing that happened when Ichigo fought Kenpachi. Unohanas bloodlust tricked his mind into thinking he'd been stabbed.
