Can't be that that simple. Its going to end up being they do something to ichigo that awakens his hollow powers and hollow ichigo killed her.

The chapter says that Isshin didn't get his shinigami powers back until after Ichigo's inner hollow awoke though, which was basically when he went to rescue Rukia. I'm certain that's the point in time he wanted it to mean since it fixes the potential plot hole of 'why didn't Isshin save Rukia/Ichigo from that hollow' at the beginning of the manga.

Unless they sealed his hollow again afterward or something, his hollow going berserk and killing his mom doesn't quite make sense to me.

My money is on Bach and Co, finding out about her potential as a Quincy/Hollow, try to kidnap her or recruit her. She fights backs/refuses while protecting Ichigo. And now, its personal. Hell can even bring Ishida and his dad into the fight now that a member of their family was killed.

I agree. He could of just reread the early chapters for concepts. The frame alone does not imply foreshadowing.

Isshin lost his powers because he had to be sealed in the human body, but Urahara never said anything about her losing her powers. If she was that strong, I'm assuming she could have easily taken out Grand Fisher, so I wonder if coming into contact with a hollow's presence awoke White which killed her and jumped from her to Ichigo.

I kinda hope not. I like the residual idea someone mentioned earlier with Ichigo receiving most of White, then Karin, and with Yuzu gaining very little of whats left. Helps to explain why Ichigo's sisters aren't their own little power-house reiatsu factories.

Masaki was killed by the quincy/shinigami bad guy (forgot his name the new badguy guy) for betraying the quincy.

In all likeliness it is going to be by the hand of the Quincies to further push the plot.

If it isn't. Could it possibly play with he fact that how Isshin said Masaki was always the center of their universe and him being a certain distance away from her weakened the bond? Highly doubt it but who knows.

Edit: What if it is Ishidas Grandma who summons the hollow out, seeing how she is so pissed off about her son not being able to continue true blood line. I get bored on the way home from yoga and think of stupid shit.
