Things like "cock holster" and "fudge packer" would be more like "pretty green hat". They don't mean what they literally say; the insult is in the special meaning known by the people using them.

Something like "May your child be born with an imperforate anus" means exactly what it says, I doubt there is any special meaning known only to native speakers.
