Good question lol, I really can hardly follow the fights in the manga but now that you mention it they never went over that. I guess he just died when all the ninja were knocked out by whatever it is that knocked them out and put some of them (e.g. Kakashi) under that rubble.

Tsunade should have gone after pain first. Stop the bleeding before you do blood transfusion. I remember kakeshi used his eye technique more than once last time he fought diedara, why is he having trouble now?

The chapter actually explains how pain gets all his bodies. I think the only way to kill him is to destroy all 6 bodies at once before he transfers his soul to new dead bodies.
