Not an answer to the "How does Madara know Nagato?" question, but Tobi did reveal to Konan that he gave Nagato the Rinnegan. So it's conceivale that Madara planned this from the time when he was alive and well, perhaps up to and including the name of the kid, and Tobi followed through after Madara's death. As to who/what Tobi is, it hurts my head to think about.

At some point, doesn't Tobi say his goal is to have a "more complete body"? This made me think that the man behind the mask might still be Madara (or some form of his consciousness), until I noted Madara's reaction from when he stepped out of the Edo Tensei coffin: The way he spoke of "That brat Nagato has really grown" and his surprised reaction at realizing it was Edo Tensei that brought him back. That shows the guy's been out of at least the recent loop of things (given the Nagato / Zombie Ninja Army events were fairly recent).

Aren't there only 2 options for who's behind the mask?
Tobi masked man has the Eternal Mangekyo comprised of Madara and Madara's brother, So doesn't that mean it could only either be Madara, who mind swapped from his original body for some reason (but that would mean he still doesnt have a soul!), or it would have to be Madara's brother.

Or I guess someone could have just taken the Eternal Mangekyo eyes and transplanted them? idk. I didn't think about that last part until after i typed that out.

Yeah, Tobi has to be either some bastardized form of Madara or his brother Izuna. Only two who can fit the description and be capable of knowing/doing everything he has done. As far as them knowing Nagato, might as well wait til Kishi explains it.

Maybe Im just being dumb, but has Tobi ever actually used/shown his EMS? I can only remember him using normal sharingan, and maybe mangekyo, I always thought it was weird he wouldn't show his EMS since they consider it basically invincible.

I'm usually one of the first ones to ramble on about magical ninja theory, but I think the information just doesn't exist in the story yet to really tie everything together.

How does Tobi have the ability to give others the Rinnegan? (and the whole jumble of other stuff that goes with that)
Who/what is Tobi? I could be remembering wrong, but when he rips off his arm in the Danzo fight in the anime, it's very white Zetsu-looking. I know he replaced it with a Zetsu-like arm, but the fact that it was seemingly originally like that is strange to me.
How is zombie Madara still young if he knows somewhat current information, Nagato, etc.? I'm betting it's something retarded.

This theory doesn't hold weight currently. Edo Tensei affixes the person's soul to whichever body the summoner users. So it has to be Madara's soul.

After failing Zetsu and Tobi, he probably realized he had to use an unborn baby to create the Rinnegan, and thus inserted the two genes (himself and that of Shodaime) in to a pregnant woman belonging to the Uzumaki clan. Why the Uzumaki clan in particular I don't know, but there's obviously something special about them. Perhaps their blood is needed to combine the two proberly and create the Rinnegan.

I actually like this theory. Seems plausible and helps to support how the hell an old fart like Madara would know about Nagato.

Madara stated he descended from the Uzumaki lineage when he saw Nagato's red hair, an apparent trademark of the Uzumaki clan, turned grey upon using the ressurecion jutsu for Konoha. It was after Madara trolled Konan into getting Nagato's eyes from his corpse.
