it's all in context. you take into account what is said right next to that paragraph, "he fell into despair" it's describing his fervored search for another with eyes like his that he could claim them.
So what if Madara isn't Madara? What if in actuality it's Madara's brother, and Madara being just a shell of his former self is an odd way to describe how that came to be?
That's a bit of a stretch.I think someone would've remembered Uchiha Madara's brother still being alive-- If you think about it, Madara would have HAD to have killed his brother after stealing his eyes to cover up what he had done, so as not to demoralize the Uchiha clan and, by extension, his leadership.
Otherwise, I'm sure they wouldn't have accepted him killed and mutilating his brother and their co-leader. From what we know, Madara was still leader of the Uchiha after stealing his brother's eyes, and became co-founder of Konoha, so... I think we can eliminate that theory.

Madara might not have been the first, but the last or first in a short trend that ended soon thereafter. At least that's what you'd gather from the idea that the practice was encouraged for decades (the lifespan of a person who started the trend). Also the tablets written that Itachi told Sasuke to read might have had something to do with the whole "killing your brother thing" which a majority of the clan might not have had access to.
Perhaps the practice was ongoing but simply covered up, which would make sense. Nobody would have multiple sons and stuff if they simply saw them as one potential strongest son (whoever killed the other after attaining his mangekyou).
About tobi... You know how madara's hair covers his right eye, and tobi's mask only shows his right eye (just like obito would be) maybe tobi only stole one of madara's eyes. HMMMMMMMMMMMMM

From what I've gathered everyone I've actually talked to about this has said "ohblahblah sasuke will do genjutsu and steal itachi's eyes" or whatever.

Then, as we saw a comment saying that this year the manga will focus more on sasuke, well, this year sasuke goes on a journey for new eyes! To defeat Itachi's new power, or steal tobi's one eye, or team back up with the leaf because hes a blind faggot who is selfcentered and hey look the story continues sasuke tries to surpass his brother.
I mean... if sasuke gets Itachi's eyes......... what is there to write about? A sharingan vs sharingan again with sasuke against madara? Or a Vs Rinnengan with Pein? Or... well who really cares sasuke's character relies on his brother. Oh well, its all just fluff and pointless until we are told that the secret of the strongest sharingan that madara has and itachi wants is that you can do fusion with them. Thats what happened to madaras brother, his eyes got stolen then the fused and madara was near allpowerful. He never really killed him, they just did a sharingan dance. I could see Kakashi giving up his Sharingan to Sasuke if Itachi does wind up getting his eyes. Whether or not he comes back to Konoha is another matter, though.
