It's not the inbreeding really that makes you more likely to have a birth defect. It's the fact that both parents may have the genes for the same disease which increases the kid's chance of having it. If hemophilia (popular Russian example) was something that was in your family, having both parent be carriers would increase the chance that their offspring would have it. However, two completely random people who aren't related, but both have the genetic carrier for hemophilia have just as much chance of having a hemophiliac offspring as related people. The big problem comes with generation after generation of inbreeding (as seen with pure-bred dogs and cats) where such a gene is passed down continuously and becomes so prevalent that pretty much 99% of the offspring of a certain group will have said genetic ailment.

Aside from the ick factor (imposed by society), there's nothing that wrong with inbreeding. The same reason people use for condemning inbreeding (genetic frailties) is similar to Hitler's idea of breeding superior humans selectively. If you took it out to it's ultimate conclusion doctors could one day tell you and your wife you are not allowed to conceive because there is an unacceptable genetic likelihood that you'll pass down something negative to your child. I guess it depends on the future of Eugenics.
