For the most part of this song there really is only repeating 3, 4, or 5 times of the same note. If you can count it out as you go its pretty easy. The notes also go pretty well with the beat, so keeping the beat with your head or feet makes it easier.

And the game will be 50 bucks I believe... not sure i'll buy it, it depends greatly on the contents of the game. They said that it won't only be Aerosmith songs, but also songs that they found were inspiring, or their favorite bands and whatnot... so it definitely has potential.

I'm still putting off learning how to alt strum, I don't want to have to learn the 5th button and a new strumming method all at the same time. So far on the first 4-5 sections of songs I have been able to keep up downstrumming but its cutting it close sometimes.

Learn to alt-strum absolutely asap or you'll regret it.
The hardest parts of the game for me right now are one's fast solo a, rb's mosh 1, and trying to get myself to properly alt-strum stuff I'm too used to down strumming. It drives me absolutely nuts.
I guess I could go back to some of the harder medium songs to learn to alt strum so I don't have to worry about the 5th button. It feels like it will be harder to keep the beat with alt strumming since upstrum doesn't seem very natural.

I agree, but you get better at it with practice of course. I only learned down strumming through all of GH2 Easy -> Medium -> Hard -> Expert (Misirlou being the first song where I was FORCED to alt), then started on Expert in GH3 and only used down strumming until Holiday in Cambodia and it only got harder from there with Knight's of Cydonia, Raining Blood, One, The Way it Ends, etc...
Just play a lot of those songs over and over and you'll eventually not suck so bad at it and even start to do it on some easier songs where it isn't necessary at all.
keep playing barracuda until you can 5 star it every time is my best advice for it
Alt strumming is fairly easy on long draw out repetition of the same note over and over again, but when its combined with fast note changing it becomes hard. Also, on notes that have an emphasis in the song, I cant remember which song but its repeating G G GR all right next to eachother at one point, I keep trying to downstrum that GR even if its on an upstrum cause of the emphasis on the note.
I also have realised I have never done a pull off before, where you have all the notes already pressed down and just lift them off, I guess I'm hammering those on as well.
