Anyone got hacked recently and have the form? I just got hacked and contacted SE. They said they would send me the form, but I haven't seen anything from them. Gonna call them again tomorrow.., but just wondering if anyone has one. Thanks!

i just got the urge to come back this past week, i had quit back in early feb. Tried to log in and my password doesnt work. called a rep, didnt have a piece of info on hand, so he froze my account, and said he would email me a form. i still haven't received it, but if/when i do ill copy and paste it here for you.

This should be the document you need. It's a .pdf file, only zipped it so I could attach directly rather than find a place to upload and have to post a seperate link. I got it a little over a month ago for my sister's account that was hacked.

Anyone know if Larenzo on Phoenix server was hacked? I seen him with a big RMT LS on our server KNUCKLEGUY. Larenzo has on Armada LS though. Just putting out info.

I think they might have changed the form. They said I don't need to get anything notarized. Or they might actually have two forms like if you don't have information on the account or it has been changed, then you need to use the form that requires notarization. None of my info was changed, not even password. Somone (not SE) just sent me one that is called "FFXI GDR Declaration". Hmm, anyway thank though, I will call them again today.

do you live in the states? if you and maacha aren't on the same continent it's completely possible the form differs based on where the player lives, because of local laws etc. While it was kind of maacha to upload that file, i would advise people reading this to hold out and respond to the specific form they recieve instead of doing it ahead of time and possibly sending the wrong form and delaying your process even further.

I'll let you guys know if the one i get differs from the form posted; i was told i would get it within 24 hours in my mailbox, but of course being SE, that's not the case thus far.

update: called a 2nd time. this rep was far more helpful. sent the form to me immediately. mine requires it be signed by a notary and a photocopy of my driver's license.

Were you able to verify the ownership e.g. have all cc info, registration code, etc all correct? I called yesterday too and the guy asked which form the other person said he would send, the notarize form or the GDR form, so I said the other guy told me I don't need anything notarized.

he only asked me for my POLID, DOB, and full name the 2nd time, although that could have been because he knew we had called the day before.
