So, at the beginning of the series, Garrison was a closeted homosexual.

Then he became openly gay. He was a male who was sexually attracted to men. ("4th Grade")
Afterwards, he underwent surgery to become a woman, who was still sexually attracted to men. ("Mr. Garrison's Fancy New Vagina")
But then, he became a lesbian, and was a woman sexually attracted to other women. ("D-Yikes")
And this episode, it's assumed he'll become a man again, who is still attracted to women.
