sneak or invisible has never failed me, only slightly new players who accidentally aggro'd a handful of mobs with low hp (but with sneak and invisible on)
Assuming sneak works as the description on sneak oils (and by extension invisible (lolingamedescriptionsbeingaccurate)) then the game generates a random number 256 (or whatever the game process time runs at) a second between 1-10000 when you have sneak/invis applied and are within a sight/sound aggressive monster. Roll a 1 and you get wtf how i get aggro'd, any of the other 9999 numbers and you're good to go. Assuming, ass, you, me etc etc, but it makes sense to me. Rare as a veritable motherfucker but has a very slim chance of happening.
Fraps or it didn't happen is what most people are thinking.

No pics, but waaaay back I once had sneak up in Eldieme and one of the ghosts (low level ones near the first switch) aggroed me when I ran by him at full hp. I don't recall if I got the "sneak wears off" right before he hit me or after, but I never got the 3 warnings.
I'm going to make a controversial statement, but...
Demons also detect sound. It's a very small radius, but they can. You can try this by standing behind an imp, right after it's stopped moving (and thus wont be moving again soon).
You can also test by running around with both sneak and invis, versus invis only.
I noticed this a long time ago while soloing demons outside of castle in Xarcabard (personal experience).
Most people tend to cast only invis around demons... tends to also explain why majority of the posts on here about catching aggro through invis are from kindreds... that aside, the smn might still be glitched through sneak/invis etc...
This one time, I ran by a mob with snk/invis up, and it hit me, and I freaked out and started cursing about snk/invis not working right.
Then it turns out one of my pt members had aggroed it before me through "normal" means and just bound it, so when I came strolling it spanked me in passing.
Ok I haven't been to the Range in years, but iirc taurs and demons are in the same overall family and will link? If not then the rest of what I write here is wrong.
A) You had S/I up but a Taur aggroed from the "edge" of normal range via true sight
B) And it was snowing so the Taur lost aggro after a bit without ever catching you
C) Before he lost aggro and was still thinking about you, a Kindred SMN linked with him and took up the chase instead?
Something like that? Or not.
This would make sense, however, there would be more reports of the Kindred Warriors and Dark Knights linking instead of just the Summoner.
In 4 years of playing I never, ever, once got aggroed when I shouldn't have through sneak/invis.
I have a picture of a Kindred Summoner aggroing me with Sneak+Invis on, it looked like this:
Summoner's Elemental hits me for x dmg
Your Sneak Effect Wears off
Your Invisible Effect Wears off
I'm not sure where the picture is, but I'm sure I have a picture of it, I'll post it if/when I find it.
Aside from that it has never failed on me, and I believe it is 100% effective, but Kindred Summoners are glitched or hack or something.
I've had the same problem with a Kindred Summoner, but only once that I can remember. Otherwise, I've never noticed anything but 100% success. People said I had probably aggro'd a taur, but I highly doubt it as the only one nearby was far around the corner at the time. Other people mentioning it makes me want to spend an hour trying to aggro Summoners, lol. If it matters, I think it actually was the elemental that attacked first, like the person above me posted.
The taurus doesn't have to be far away. Quick example: You aggro one of the DC Tauri without noticing it (I always stop now at the first tunnel past the slope with buffalo and raptors to check for this). Because of its lousy pathing and/or your movement speed bonus, you get far ahead by the time it wanders in front of a demon. The demon takes off after you at running speed, but they give up fairly quickly and you have enough of a head start that it deaggroes and despawns. However, before it deaggroes it runs in front of another demon, that takes off after you. Repeat as many times as is necessary until one of the demons catches up and rapes you.
Seriously, doesn't it strike you as strange that the _only_ mobs that experienced players can report "aggroing" through sneak/invis are in this one zone where really fast mobs link to slow mobs which are true sight?
Screenshot here: I had previously died to a minotaur, so I was still weakened but being EXTRA SUPER CAREFUL. There were no Minotaurs anywhere remotely close, and I didn't get any "The effect of ... is about to wear off" messages. That said, I'm pretty sure Week's post above got it right, and this was a link to a minotaur somehow.

Not saying you're completely wrong, or that I'm 100% certain I actually aggro'd the Kindred SMN, but I'm fairly sure that tauri are as fast if not faster than normal running speed (I don't have movement speed+ on any of my jobs at the moment, save for BRD, which I didn't have leveled at the time) so if anything it was a nearby aggro and not a long link of them. I just think it's odd that most (or all?) of the people who say they got attacked by a demon with Sneak/Invis up and didn't see any tauri nearby were all attacked by the same job, which leads me to believe that it's more of a glitch. I don't think that every single person here who said it happened is so oblivious that they all happened to link by tauri that were nowhere in their sight range, or that no matter what their race/job/buffs/etc. the demon killed them before the taurus even hit them once or got close enough for them to see the aggro (again assuming that tauri are same speed/faster than PCs, which I'm fairly sure they are). Maybe so, but I don't think a glitch is out of the question.
It's probably a combination of aggroing tauri, and the smn's elementals linking and aggroing immedietly, as opposed to linking with the tauri than following.
Personally, I've ran circles around the kindred smn on the way to ENMs after reading about it on BG, and never got aggro. And on the topic of tauri run speed, I have no idea if they actually run at the same speed as a player without +movement, but with pathing they definitely run way slower. They fall behind me and out of sight pretty quickly just running in a straight line...

I've had the same thing happen to me with the smn pet's on the way to the enm before in ugly range. It can't be a glitch, because we all know the dev team at SE is perfect, and they test everything forward and backwards before releasing it. Ok /sarcasm off.
This has never happened to me, but it ALWAYS happened to my friend when we went to duo Brothers.
She GM'ed about it after like the 5th time, and they finally explained it. Apparently, in one of the seemingly "safe" areas (you know, the places right before a tunnel, after a long stretch of aggrorific area), there is a spot that demons on the ridge above can see you, and will aggro you. And run the long way, ending up to you like...15 minutes later.
That's what the GM said at least /shrug
