I'm going to go ahead and say it's always been IEXPLORE. Are you confusing it with Explorer.exe?

I can only see ONE instance of iexplore. (unless it's running and you're using Firefox, while IE is closed)

    I think you are fine.

You're mistaken. =\ It's always been iexplore.exe for Internet Explorer, and explorer.exe for the Windows browser.

    EDIT:
    Source 1
    Source 2
    Source 3
    Source 4

Etc, etc. Looks like you've been living with some nasty stuff on your PC and you've thought it was normal.
