Try a different set of speakers, and see if the same thing happens, and/or borrow a soundcard and see if it still happens.

He mentioned that this also happens with his headphones, so it doesn't seem to be specific to the speakers.

Some of the first things you can try are:

    1. Playing different types of files (e.g., a CD if you've only been playing MP3s).
    2. Opening the Windows Volume Control and adjusting "Volume Control" and "Wave" separately while playing something.
    3. Also within Windows Volume Control, muting "Line-In" and/or "Microphone."
    4. Unplugging your microphone (whether it's analog or USB).
    5. Downloading a new set of sound card drivers, uninstalling your old ones (do this in Control Panel -> System -> Hardware -> Device Manager), and then installing anew.

Do any of those fix the problem?

This happened to me, and it turned out to be my on board sound card. Bought a new one relatively cheap, that works really well ($20? $30?) and it fixed it.
