Honestly, while there are many in-game issues people have with the game still (AV and PW being the most glaring), SE has made huge, if slow, advances in improving just about everything about the game itself and it's support. They have at least started listening even if they still have an incredibly patronizing attitude about game design.
The only time you get the "few vocal complainers" response is when you get the child-like responses that you see on places like game forums. If people actually take the time to fill out the survey, or submit feedback through the legitimate channels in decent grammar and with intelligence, it would difficult to ignore the real complaints people have.
The problem is the we are playing (I am not even sure how much you play anymore) a game with a relatively large population of children and young adults. Although even when I was young I never acted in a manner close to some of the worst whiners on BG, this younger group seems to be unable in many cases to act in a mature manner when dealing with making proper responses to actually issues, in game or in real life in general. This is not limited to FFXI at all, nor to BG, it's just a general observation of the generation after mine. As much as I complain about BG sometimes, it is a paragon of calm and placidity compared to some other game forums, e.g. WoW.
