If this really happened, wouldn't everyone in the alliances receive the message "So and so recieves the <longbow> fragment" ? so, everyone would have gotten the message that two people got fragments.
Also, that GM convo doesn't prove anything, sorry: Then again, i've only been doing dynamis for over 4 years, and its never duped on the dozens and dozens of people that have DC'd in the past. This is just my opinion, which isn't really worth much outside of my own moghouse, an since it seems like it was a one time only situation, it doesn't matter much I guess.
There are so many bugs like that in FFXI. Not long ago I entered salvage (uhoh!) and basicly most of my instruments were gone and some of my other equipment pieces. Relogging fixed this issue. Another similar one is when you use a cell in salvage and the equipment doesn't unlock for you unless you again relog to fix the issue. Or when you put something in bazaar and close too quickly you will have 'locked' the item (as if it were in bazaar) but not have it in your actual bazaar.
It's pretty much impossible to say exactly what happened here since we don't know enough background information about the supposed bug to 'fill in the blanks' so to speak. It does remind me somewhat of the trade bug that was patched ages ago where you could d/c while trading to basicly dupe the item you were trading.
Oh, nobody's arguing that point, to be sure, but I seriously doubt this is due to instance inheritance issues like daedalus stated. The last paragraph is an interesting one because I'm curious of that myself.
I think it's less of an issue of merging new code in with old, and more just making global changes in multiple places without considering all the places the change is being made.
On the other hand, maybe...just maybe...

I DC'd during a DL fight last week. When I got back I saw DL drops in pool, but also saw the winning lots. I guess im not unlucky enough to get duped items.
This; I had one just the other night where a good 10-12 pieces of my inventory were just...missing...as I zoned out of Salvage. Logging out corrected it (god damn if it didn't scare me though, I had a good 10m in gear in the disappeared stuff!), and there was really no other indicator of what might cause it...didn't d/c, didn't r0, zoning didn't take any longer than usual...hooray software glitches.
Why was this unlocked? I don't see any pics besides the one Sath posted.
And a GM convo doesn't really prove anything. Now if you had two pictures from each of the fragment obtainers with the fragment in both of their inventories, I'd start thinking it could be true.
EDIT: Oh yeah need names too. "Some guy in my LS said it happened to him" doesn't count. That SS from Sath looks like the intended reciever of the fragment who lotted it. He was probably calling the GM to make sure his shit is safe. The "duper" is just fucking with you all.
Unless it really does involve some massive recent code changes to make Dynamis ready to be instanced, I'm calling bullshit too. I've been doing Dynamis for years, I've DCed a lot more than most (used to have really bad crash problems in Dynamis) and I've never seen anything other than the usual "one treasure pool for the whole zone regardless of whether you're in alliance" behavior. You'd literally have to dupe the whole Dynamis zone server to get multiple loot pools, which would take a bit more than DCing during a fragment NM.
I have had the disappearing gear thing happen in Dynamis, which scared the living shit out of me because it made my BLM AF2 hat, four HQ staves and a bunch of other random gear go poof. Logged out and back in and everything was back.
