Are you looking for a gourmet burger, from a resturaunt/chef/homemade? Or are you looking for a Burger from a Chain?
Best burger I've had from a fastfood place was from FiveGuys. I don't go much cause it's expensive for "fast food" coming to around 20$ for 2 people usually, but I can't resist a double cheeseburger with bacon lettuce tomato and mayo from them. Best quick burger around.
I have never had a burger from any resturaunt chain that I've really enjoyed, although there are only a few places I'll even get steak at anymore for less than 100$ a plate. If your looking for outrageous things you can do-yourself at home and such, then I can't really help you. The ones my family makes, and my sister make are pretty damn simple and are by far the best tasting i've ever had.
I saw on the cooking network a guy that put not only spices, but cheese and bacon into the ground beef before even making the patty, so that there was cheese and bacon INSIDE of the burger. Looked amazing but never tried it myself.
i lost my 5 guys virginity recently and while it was indeed amazing, the 14 dollar bill for a hamburger fries and coke pissed me off
bun: corn dusted kaiser roll
meat: 3/4 lb or ground deer meat mixed with 1/4 lb of reg ground chuck(the beef allows it to have lil juicier flavor)
also in the mix chunks of velveeta cheese, greek seasoning, bacon, hot sauce, minced garlic and chopped onions.
toppings: siply add your favorite chees(i suggests colby jack), a pickle or two, mayo and ketchup.
p.s. mind you this is one pretty damn big burger. but, your taste buds will thank me
Five guys burgers and fries serves up excellent burgers and fries. If you are ever in the New Haven Connecticut area stop into "The Educated Burgher" they serve up some amazing beef.
I'm gonna add a +1 to the Five Guys suggestion. Their burgers (and fries, for that matter) are the shit.
However, if I'm at a real fast food chain, it's either the Baconator or the BK Quadruple Stacker.
I think they have In N' Out in Utah, Mojo? I'm from cali but that's where I always go... Because I can tell them to grill my onions. Double double with grilled onions+animal style fries is the shit.
In terms of gourmet sit down places, Islands or Red Robin is great. If I make a burger I always put bleu cheese in the meat though!
Only been there once, but In 'N' Out is great, too. I prefer Five Guys, though.

I was able to visit Five Guys after reading this thread. It was pretty good although not good enough to end the search. I'll probably go back soon though, I brought the burger home in a bag (doesn't quite do them justice that way.) They're building an In n Out burger joint about 35 miles from where I live, when it's done I'll have to journey over there to check it out.
Five Guys is by no means the ultimate burger, but I will say that it is far and away the best out of the bigger restaurant chains.

5 guys is good, although it is nowhere near great solely because they will NOT cook your burgers medium rare unless they are in one of those 'we don't give a shit' days. Their ingredients are good, and so is the service, but the corporate policy about burgers being well done just ruins it.
Also, their fries are no where near the best. They don't do the double fry technique (cook at 2 different temperatures) therefore they are nowhere near the best.
If you live in the Chicago area, I suggest checking out Beinlich's (It's in Northbrook adjacent to US41/I94). It's not a chain restaurant, but holy fucking hell it's the god damnedest good burger (probably not THE best, but you know what I mean). They also close randomly during fishing season when the biting is good, lol.
I need to try this FiveGuys place, but I've never heard of one or seen one around where I live.
